Onside are delighted to be working in partnership with Ombersley Medical Centre to deliver the exciting new Neighbourhood Social Prescribing service.
Social Prescribing recognises that may of life’s problems can make you unwell or make health conditions harder to deal with. Medical interventions and treatment may only be part of the solution. Social Prescribing can provide other options by linking patients with sources of support within the community to improve their health and wellbeing.
The Social Prescriber will work with patients to explore what is important to them and what are the issues, difficulties and challenges they face. They can then support the patient to access services and activities in their local community. This can include a wide range of options, for example, advice and information on housing, finance and benefits support in to education and learning, healthy lifestyle advice and social activities and mentoring.
Patients can speak to their GP or another member of the practice team to get a referral to the Neighbourhood Social Prescriber.
