Worcester A&E/OPAL Team – for information

In most situations, when a frail or elderly person is taken to A&E they want to return home as soon as possible. A&E staff will liaise with the OPAL team whose key focus is older people with frailty. The team will try and ensure the patient is not admitted if necessary and is given intensive input to minimise their length of stay by working with the team at A&E to ensure the patient has access to the help they need. There is pharmacy representation in the OPAL team who need to know the patients usual medication and are keen to liaise with patients/carers/practices and community pharmacists when arranging discharge.
